this Form be completed and returned to us by the stipulated date to ensure the insertion of this
information in the Catalogue. For unreturned forms and late submissions, the organizer reserves the right to edit your company listing according to other information provided for promotional
use OR "PLEASE VISIT OUR BOOTH FOR FURTHER INFORMATION" will be listed under your Company's Name.
